What your day will look like:
- Perform all core Medical Assistant duties, including rooming patients, obtaining vitals, preparing charts, and supporting providers during visits.
- Serve as the central coordinator for the Memory Care Program, helping establish processes, communication pathways, and patient?support workflows.
- Work independently to identify gaps, troubleshoot issues, and implement solutions that improve patient experience and program efficiency.
- Support neurologists and clinical staff by gathering relevant patient information, assisting with cognitive?care protocols, and ensuring accurate documentation.
- Act as a resource for patients and families, helping them understand next steps, referrals, testing, and follow?up expectations.

To be a champion in this role, you will need:
- Experience as a Certified Medical Assistant or LPN, preferable with exposure to geriatric population.
- Interest in cognitive disorders and a desire to support patients with dementia, Alzheimer’s disease, and related conditions.
- Demonstrated leadership qualities and interest in program development.
- Compassion, patience, and a genuine desire to work with cognitively impaired older adults.
