Ascendo Healthcare Staffing is actively seeking an experienced Home Health Registered Nurse, RN for an exciting 13-week contract opportunity near Penobscot County, ME ! This opportunity is moving quickly, and interviews are being scheduled immediately!
NOTE: This position is specifically for a skilled Home Health RN. Candidates whose experience is limited to Home Care, private duty, sitter care, continuous care, or non-skilled support work may not be the right fit. Qualified candidates must have experience with clinical assessment, case management, OASIS, and home health documentation.

Required Qualifications:
- Active RN License Maine or Compact Nursing License
- Minimum 2+ years of recent Home Health experience (Geriatrics ONLY)
- Homecare Homebase (HCHB) experience
- OASIS documentation experience within the last 3 years
- Strong clinical assessment and critical thinking skills
- Excellent customer service and communication skills
- Reliable transportation and strong time management abilities
- Professional, compassionate, and patient-focused approach

POSITION DETAILS
- Position: Home Health Registered Nurse (RN)
- On Call: Rotation Required (Typically every 3rd or 4th Weekend)
- Contract Length: 13 Weeks
- Start Date: 7/13/2026 or as soon as cleared
Compensation:
- Earn up to $126,880 per year PLUS Mileage Reimbursement
- Weekly Pay: Via Direct Deposit Every Friday Morning
- Benefits: Medical, Dental, Vision & More (Single & Family Plans Available)
Schedule:
- 5x8 Day Shifts (8:30 AM – 5:00 PM)
- On-Call Expectations: Rotation Required
- Productivity Expectations: 30 Units Per Week =40 HRS
- Note: Our client is looking for Home Health RN's that can maintain a full caseload of 30 productivity points per week. No more guessing! Full-Time Hours!
RESPONSIBILITIES
- Complete comprehensive patient assessments and develop individualized care plans
- Manage a home health caseload while maintaining quality outcomes and productivity standards
- Perform Start of Care (SOC), Resumption of Care (ROC), Recertifications, and OASIS documentation
- Coordinate care with physicians, therapists, caregivers, and interdisciplinary teams
- Provide patient and family education regarding disease management and treatment plans
- Monitor patient progress and communicate changes in condition appropriately
- Participate in on-call triage rotation as assigned
- Maintain accurate and timely electronic documentation utilizing Homecare Homebase (HCHB)
