Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in providing support and guidance to individuals and families in need, ensuring they receive the necessary resources and services. This position requires a strong understanding of child welfare, behavioral health, and crisis management, as well as the ability to work collaboratively with various stakeholders to promote positive outcomes.
Responsibilities
- Conduct assessments to determine client needs and develop individualized service plans.
- Provide crisis intervention and support to clients facing immediate challenges.
- Facilitate access to resources such as housing (including Section 8), healthcare (including Medicare), and behavioral health services.
- Utilize motivational interviewing techniques to empower clients in their decision-making processes.
- Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
- Collaborate with multidisciplinary teams, including social workers, addiction counselors, and other professionals, to coordinate care.
- Maintain accurate documentation of client interactions, service plans, and progress reports.
- Supervise case management interns or support staff as needed.
Experience
- A degree in social work, psychology, or a related field is preferred.
- Experience in child welfare or behavioral health settings is highly desirable.
- Proven skills in crisis management and intervention strategies.
- Familiarity with addiction counseling practices is a plus.
- Strong communication skills and the ability to build rapport with diverse populations.
- Ability to work independently while also being part of a team-oriented environment.
- Previous experience supervising staff or interns is beneficial but not required.
