Job Summary
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Caregiver to join our team in providing exceptional care to individuals across various care settings. Responsibilities
- Assist residents and patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient conditions, recording vital signs such as blood pressure, pulse, temperature, and respiration accurately in electronic health record (EHR) systems
- Support medication administration under supervision, ensuring proper documentation following home healthcare regulations and HIPAA guidelines
- Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies
- Assist with medical procedures such as catheter care, wound dressing changes, and hospice/end-of-life care needs
- Support safe transfer techniques using equipment like Hoyer lifts or other assistive devices while adhering to safety protocols for heavy lifting and patient handling
- Maintain a clean environment by assisting with laundry, cleaning duties, meal preparation, and ensuring infection control standards are met
Requirements
- Valid certification as a Nursing Assistant or Caregiver with active state licensure or registration
- Proven experience working with geriatric patients in assisted living facilities, nursing homes, or home healthcare settings
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Knowledge of medical terminology relevant to patient care and medication administration procedures
- Experience working with individuals with developmental disabilities or disabilities requiring specialized attention
- Ability to perform CPR and basic life support (BLS), with current certifications preferred
- Strong understanding of home health regulations, HIPAA compliance, and safety protocols for hospice and end-of-life care
- Excellent patient observation skills to detect changes in condition or behavior promptly
- Physical ability to perform heavy lifting tasks safely and assist with transfers using appropriate equipment such as Hoyer lifts
- Compassionate caregiving attitude combined with excellent communication skills to work effectively within a team environment
