Job Summary
Summit Healthcare Staffing is hiring on behalf of a client who iseeking a compassionate and detail-oriented Registered Nurse (RN) to join their Intake team at a growing home health organization in Brooklyn, NY. This is an excellent opportunity for an experienced nurse who enjoys coordinating patient care, collaborating with interdisciplinary teams, and ensuring patients receive a seamless start to home health services.
This position begins on-site in Brooklyn, with the opportunity to transition into a remote role based on performance and training.
Responsibilities
- Review and process incoming home health referrals.
- Verify patient eligibility, insurance coverage, and required authorizations.
- Coordinate timely admissions to home health services.
- Ensure referrals meet clinical and regulatory requirements.
- Communicate with referral sources, physicians, clinicians, and patients to facilitate a smooth admission process.
- Collaborate with the Billing and Authorization teams to ensure accurate documentation.
- Maintain compliance with agency policies and state and federal regulations.
Qualifications
- Active Registered Nurse (RN) license required.
- Previous experience in home health, intake, case management, or care coordination preferred.
- Strong organizational and communication skills.
- Ability to multitask in a fast-paced environment.
- Experience with insurance verification and authorizations is a plus.
- Detail-oriented with excellent clinical judgment.
Compensation- $100,000-$120,000
Pay: $100,000.00 - $120,000.00 per year
