Position Summary
Prestige Concierge Nursing is seeking a compassionate, highly skilled Licensed Vocational Nurse (LVN) to provide one-on-one concierge nursing care for a medically complex pediatric patient in the home setting. The ideal candidate is clinically competent, detail-oriented, and passionate about delivering exceptional family-centered care while maintaining the highest standards of patient safety and professionalism.
The patient is a child with a traumatic brain injury (TBI) requiring 24-hour skilled nursing care, including tracheostomy and mechanical ventilation management, gastrostomy tube (MIC-KEY button) care, and total assistance with activities of daily living.
Essential Responsibilities
Respiratory Care
- Provide continuous assessment and monitoring of respiratory status.
- Care for and maintain a tracheostomy.
- Monitor and manage mechanical ventilator settings as directed.
- Perform tracheal suctioning utilizing sterile technique.
- Administer supplemental oxygen per physician orders.
- Recognize and respond appropriately to respiratory distress or ventilator emergencies.
- Perform routine tracheostomy care and trach changes as directed.
- Ensure emergency airway equipment is immediately available.
Gastrointestinal & Nutrition
- Administer enteral feedings through a MIC-KEY button G-tube.
- Administer medications via G-tube.
- Monitor feeding tolerance and hydration status.
- Perform routine G-tube site assessments and care.
- Report concerns such as leakage, dislodgement, infection, or skin breakdown.
Neurological Care
- Perform ongoing neurological assessments.
- Monitor for seizure activity and implement seizure precautions.
- Assess for changes in level of consciousness and neurological status.
- Report significant changes immediately to the RN and physician.
Personal Care
- Provide complete assistance with activities of daily living.
- Perform incontinence care while maintaining dignity and skin integrity.
- Reposition patient at scheduled intervals to prevent pressure injuries.
- Perform range-of-motion exercises as ordered.
- Maintain excellent infection prevention practices.
Medication Administration
- Administer medications accurately according to physician orders.
- Monitor for medication effectiveness and adverse reactions.
- Maintain accurate medication documentation.
Patient Monitoring
- Monitor and document:
- Vital signs
- Oxygen saturation
- Respiratory status
- Pain and comfort
- Intake and output
- Skin integrity
- Overall condition
- Identify subtle changes in patient status and escalate concerns promptly.
Equipment Management
- Safely operate:
- Mechanical ventilator
- Pulse oximeter
- Oxygen equipment
- Suction machine
- Feeding pump
- Troubleshoot equipment issues.
- Maintain emergency supplies and equipment readiness.
Documentation
- Complete accurate and timely nursing documentation.
- Document assessments, interventions, medications, treatments, and caregiver communication.
- Report significant changes immediately according to agency policy.
Family Support
- Build trusting relationships with the patient and family.
- Provide compassionate, respectful care.
- Reinforce education provided by the RN regarding equipment and care plans.
- Support family goals while maintaining professional boundaries.
Collaboration
- Communicate effectively with:
- Director of Nursing
- RN Case Manager
- Physicians
- Therapists
- Durable Medical Equipment providers
- Family members
- Participate in care conferences as requested.
Required Qualifications
- Current California Licensed Vocational Nurse (LVN) license in good standing.
- Current CPR certification (BLS).
- Pediatric nursing experience preferred.
- Home health or private duty nursing experience preferred.
- Experience with:
- Tracheostomy care
- Mechanical ventilation
- G-tube/MIC-KEY button care
- Suctioning
- Oxygen therapy
- Ability to recognize and respond to emergencies.
- Excellent communication and documentation skills.
- Reliable transportation.
- Ability to pass background check and drug screening.
Preferred Qualifications
- Pediatric ventilator certification or demonstrated ventilator competency.
- Experience caring for children with traumatic brain injuries or neurological disorders.
- Experience with seizure management.
- Experience in concierge or high-acuity private duty nursing.

- Ability to sit, stand, bend, kneel, and reach for extended periods.
- Ability to respond quickly during medical emergencies.
- Visual and auditory acuity sufficient to monitor ventilator alarms, oxygen saturation, and changes in patient condition.
