Position Responsibilities
- Provide skilled physical therapy services under the supervision of a licensed Physical Therapist and in accordance with physician orders and the plan of care
- Assist patients with therapeutic exercise programs focused on mobility, balance, strength, and functional independence
- Educate patients and caregivers on home safety, fall prevention, and prescribed exercise programs
- Communicate patient progress and concerns with the supervising Physical Therapist and interdisciplinary team
- Complete accurate and timely documentation in WellSky/Kinnser EMR
Qualifications
- Active Florida Physical Therapist Assistant (PTA) license
- Home health experience preferred; training may be available
- Reliable transportation and a valid driver's license
- Strong communication and interpersonal skills
- Ability to work independently while collaborating with the clinical team
