Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor and document vital signs accurately, including blood pressure, pulse, respiration, and temperature, following medical terminology standards.
- Support medication administration under supervision, ensuring proper documentation in electronic health record (EHR) systems while maintaining patient confidentiality in compliance with HIPAA regulations.
- Observe patients closely for changes in condition or behavior, especially in memory care or Alzheimer’s/dementia settings, and report findings promptly to healthcare professionals.
- Provide specialized care for clients with developmental disabilities or those requiring end-of-life support within hospice or palliative care frameworks.
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home healthcare regulations.
- Assist with medical procedures like catheter care and wound management as trained; utilize CPR and first aid skills during emergencies to ensure patient safety.
Skills
- Strong patient observation skills with the ability to recognize subtle changes in health status.
- Proven experience working with geriatric patients or in assisted living environments; familiarity with long-term care settings preferred.
- Knowledge of home health care assistance protocols and medical documentation practices using EMR/EHR systems.
- Excellent communication skills for engaging effectively with clients, families, and interdisciplinary teams while respecting patient privacy and confidentiality.
- Ability to perform physical tasks including heavy lifting safely and efficiently; experience working with mobility aids such as Hoyer lifts is a plus.
- Competence in vital signs measurement, medication administration, catheter care, and basic life support (BLS/CPR).
- Understanding of home & community care regulations along with hospice or dementia-specific care techniques to deliver compassionate support tailored to individual needs.
