As a key member of our home health services, you will collaborate with multidisciplinary teams to deliver comprehensive care that aligns with health regulations and best practices.
Responsibilities
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine individualized care needs.
- Develop and implement tailored care plans in collaboration with physicians, therapists, and other healthcare professionals.
- Manage patient cases through clinical case management techniques, ensuring timely interventions and discharge planning.
- Administer medications, wound care, IV therapy, and other treatments following strict safety protocols and medical terminology standards.
- Utilize Electronic Health Records (EHR) systems for accurate documentation of patient records, medical records management, and compliance with HIPAA regulations.
- Educate patients and their families on disease management, medication adherence, and health promotion strategies to foster self-care.
- Coordinate with healthcare teams to facilitate diagnostic evaluations, post-acute care transitions, and hospice or palliative services as needed.
Qualifications
- Valid RN license with current registration in the state of practice.
- Critical care experience preferred, including ICU or emergency room experience; Level I Trauma Center experience is a plus.
- Demonstrated knowledge of hospital medicine, infectious disease care, geriatrics, behavioral health, and medical-surgical nursing.
- Experience in home health settings or post-acute care environments is highly desirable.
- Strong understanding of health regulation policies, utilization management, discharge planning, and case management principles.
- Proficiency in EMR/EHR systems for documentation and patient record keeping.
- Excellent patient service skills with the ability to communicate effectively with diverse populations.
- Ability to manage patients as a nurse case manager while collaborating seamlessly with healthcare professionals to optimize outcomes.
- Familiarity with medical terminology, physiology concepts, HIPAA compliance standards, and outpatient or clinic experience is advantageous.
