The Registered Nurse Assisted Living Float RN is a dynamic clinical leadership position responsible for delivering post-acute care, complex clinical assessments, and care coordination to residents across Assisted Living Facilities (ALFs) and Adult Care Facilities (ACFs). This role floats across sites within the Five Boroughs of New York City and the Hudson Valley region, overseeing LPNs, managing care plans, and ensuring residents receive comprehensive home healthcare services.
As an essential member of the Certified Home Health Agency (CHHA) team, the Float RN conducts skilled nursing visits, manages physician-prescribed treatments, administers medications, performs wound care, and educates residents and families. This position requires exceptional clinical adaptability, dependability, and full schedule flexibility across variable facility shifts.

Salary Range: $138,000 / year (Built-in mileage allowance included)
Retention Bonus: $15,000 Bonus paid upon completion of 15 months of continuous employment
Hours / Schedule: Monday – Friday (Shifts vary by facility between 7:00 AM and 8:00 PM; common schedules include 7:00 AM–3:30 PM, 8:30 AM–5:00 PM, 7:00 AM–5:00 PM, and 7:00 AM–8:00 PM). Candidates must demonstrate full flexibility for all shift assignments.
Location & Commute & Office Locations
- Training Site: Brooklyn Office (2 days per week during initial training).
- Coverage Territory: Five Boroughs of NYC (Manhattan, Bronx, Brooklyn, Queens, Staten Island) and the Hudson Valley region.
- Setting: Assisted Living Facilities and Adult Care Facilities across assigned territories.
Travel & On-Site Expectations
- Float Expectations: Daily or routine travel to floating facility assignments across the assigned coverage region.
- Shift Availability: Unrestricted flexibility to cover variable morning, day, and extended evening facility shifts.
Key Responsibilities
- Clinical Assessment & Skilled Nursing Care
- Administer skilled nursing interventions, physician-prescribed medications, and palliative care treatments to residents in adult care facilities.
- Conduct initial, physical, and ongoing health evaluations; monitor, chart, and report changes in resident conditions and vital signs.
- Perform complex wound care, assess healing progress, and manage dress/redress procedures.
- Evaluate residents for assistive tools, devices, or clinical resources to enhance independence and quality of life.
- Care Coordination & Interdisciplinary Leadership
- Oversee, guide, and supervise on-site LPNs to ensure clinical care plans are executed accurately and safely.
- Collaborate directly with Physicians, Physical Therapists, Occupational Therapists, and multidisciplinary team members to align care plans.
- Educate residents, families, and facility staff on proper home healthcare procedures, disease management, and wellness strategies.
- Documentation & Regulatory Compliance
- Maintain accurate, timely, and complete electronic documentation for all resident assessments and clinical interactions.
- Ensure all nursing practices adhere strictly to state regulatory standards, facility guidelines, and agency policies.
Qualifications & Experience Requirements
Required
- Licensure: Current, unrestricted Registered Professional Nurse (RN) license in New York State.
- Experience: Minimum of 1 year of nursing experience in assisted living, long-term care, skilled nursing, home care, geriatrics, or a related clinical environment.
- New Graduates: New graduate RNs are accepted if they possess relevant and recent clinical care experience.
- Technical Skills: Computer literacy, electronic documentation competency, and strong clinical skills.

Compensation & Benefits
- Salary: $138,000 per year (includes built-in travel/mileage compensation) + $15,000 Retention Bonus (paid after 15 months).
