MINIMUM REQUIREMENTS:
EDUCATION:
HS Diploma or GED required, Bachelor’s degree in Human Services or related field preferred.
CERTIFICATION/LICENSURE:
Community Health Worker (CHW) certification or ability to obtain CHW certification within 6 months of hire.
EXPERIENCE:
Two years or equivalent of related experience working with individuals with behavioral health disorders preferred
SUMMARY OF RESPONSIBILITY:
Community health worker (CHW) will provide support to individuals receiving services under Behavioral Health Home (BHH) benefit. Responsibilities will include working with multidisciplinary team to provide services and supports that are beneficial to the individual receiving services, assist with linking individuals to health and social services in the community, coordinating and providing access to individuals and family supports to local community supports and resources, coordinate care with medical providers, identify and reduce barriers for individuals served to increase access to care and supports, provide home and community based visits as needed, provide transportation of individuals served to resources and appointments as necessary, participate in initial and annual care plans as required through BHH benefit and assist with carrying out tasks related to care plans. Pay: $18.34 - $25.87 per hour
