Hours: Monday - Friday 8am - 4:30pm (occasional weekend and evening hours)

This position works with individuals experiencing homelessness in a shelter setting and is responsible for supporting their transition from shelter to stable, permanent housing. The Case Manager collaborates closely with community partners to coordinate services and participates in joint efforts to help participants complete housing assessments, enroll in the Oregon Health Plan (OHP), obtain documentation required for housing and benefits, and connect to local resources. The role also includes attending community meetings, contributing to coordinated community responses, and participating in outreach efforts as needed. The Case Manager may focus on one or more service areas, including case management for young adults ages 18–24, case management for singles, couples, and families, and/or community health work that integrates health and housing supports.
Preferred Qualifications:
Experience working with individuals experiencing homelessness, chronic illness, behavioral health challenges, and/or substance use disorders preferred.
Data, Documentation, and Compliance
Maintains thorough, accurate, and timely documentation of participant interactions, services provided, and progress toward goals.
Submits required data, case notes, and reports in accordance with program, organizational, and grant requirements.
Coordinates medication pickup and delivery for participants as needed, ensuring secure handling, accurate completion of receipts/forms, and adherence to established C@P procedures.
Build relationships and trust with participants
Supports participants in taking next steps toward stability by connecting them to resources, including OHP, domestic violence services, substance use treatment, and behavioral/mental health services.
Assists participants in pursuing personal goals, communicates progress with the Site Manager and Case Management/Peer team as appropriate, and helps address barriers while honoring participant dignity, choice, and self-determination.
Provides individual and group education on life skills (e.g., appointment management, nutrition, hygiene, and navigating systems independently).

Cultivating the Mission and Values of C@P Through Community Partnerships and Collaboration
Participates in collaborative efforts with partner organizations (e.g., Salem Housing Authority, HIV Alliance, Easterseals, ARCHES, Northwest Human Services, etc.).
Collaborates respectfully with all team members, aligning with C@P values—seeing with compassion, building relationships, practicing empathetic listening, and pursuing peacemaking—while applying an outward mindset and Nonviolent Communication principles.
