Responsibilities
- Conduct comprehensive evaluations of patients’ physical conditions, including assessments of physiology, anatomy, and medical history.
- Develop and execute tailored care plans utilizing evidence-based physiotherapy techniques such as trigger point therapy and orthopedics.
- Provide hands-on patient care through therapeutic exercises, manual therapy, and modalities to promote healing and functional improvement.
- Manage discharge planning processes to ensure seamless transitions from hospital or inpatient settings to outpatient or community care.
- Document all clinical interactions accurately in electronic health records (EHR) systems following clinical documentation standards.
- Collaborate with multidisciplinary teams to coordinate patient care across inpatient, outpatient, pediatric, geriatric, neurology, orthopedics, and home health services.
- Educate patients and their families on physiotherapy exercises, injury prevention strategies, and health maintenance practices to support long-term wellness.
Requirements
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home & community care settings.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
- Experience working with geriatric patients and pediatric populations to address diverse rehabilitation needs.
- Demonstrated ability to develop effective care plans for patients with orthopedic injuries, neurological conditions, or post-surgical recovery.
- Acute care experience in inpatient hospital environments is highly desirable.
