ABOUT THE ROLE
We are looking for a skilled and compassionate Registered Nurse to lead patient care coordination for our home health program. In this role, you will manage the full patient lifecycle — from admission through discharge — while supervising a team of LPNs and Home Health Aides. WHAT YOU'LL DO
Conduct initial home visits and perform ongoing patient assessments to evaluate needs and progress
Initiate and manage individualized plans of care in accordance with physician orders
Observe, document, and monitor patient symptoms, reactions, and progress
Educate patients and caregivers on diagnoses, medications, treatment plans, and home care techniques
Coordinate and supervise LPNs and Home Health Aides, including on-site supervision
Communicate changes in patient condition to physicians, the DON, PT, and case managers
Lead care conferences to ensure coordination across the clinical team
Discharge patients from skilled nursing services when criteria are met
Maintain accurate, timely clinical records in accordance with agency policy
Stay current through continuing education, in-services, and self-study programs
Follow all Florida state regulations and agency policies and procedures
WHAT WE'RE LOOKING FOR
Active Registered Nurse (RN) license in good standing with the Florida Board of Nursing
Minimum 1 year of supervisory or managerial experience in home health, hospice, hospital, or nursing facility settings
Strong interpersonal skills — ability to build positive relationships with patients, families, and staff
Excellent oral and written communication skills with strong clinical documentation practices
WORK ENVIRONMENT
This position involves regular travel to patient homes throughout the service area, with administrative responsibilities performed in a general office environment. The role requires the ability to lift and maneuver as needed, communicate clearly, and operate standard office and clinical equipment. Reasonable accommodations are available.
