We are a care home placement agency working with various Managed Care Plans as a California Advancing and Innovating Medi-Cal (CalAIM) Community Support (CS) Provider for Skilled Nursing Facility (SNF) Diversion and Transition to Assisted Living. In California, assisted living homes, also known as residential care facilities for the elderly or RCFEs, are either small 4-6 bed residential board and care homes or large 100+ bed assisted living homes. The CS is a Medi-Cal waiver program similar to the Assisted Living Waiver (ALW).
1) The CalAIM CS social worker position involves in-person visits to members for Individual Service Plan (ISP) assessments to determine the tiered level of care. This is a two hour process paid at $45/hour ($90 for the two hours).
We will also reimburse $20 for daily gas expenses
The job hours are very flexible but should be do during the weekday.

Job Requirements:
The social worker must have a Master's degree in social work plus one year of related work experience.

Be familiar with computers and data entry
