Collaborative Living provides 24-hour residential care and supervision to Oregon people with intellectual disabilities/developmental disabilities who represent the most risk to the public at large, their peers, or themselves. Collaborative Living provides residential care, which includes health and medical care, behavioral support, job and training support, personal care, education, recreation, psychological services, and community integration. Collaborative Living is dedicated to providing the support necessary to maintain the quality of life, achieve the highest possible level of independence and promote social opportunities, which benefit the individuals and the community. Each individual residing in the program has an Individual Support Plan (ISP) that emphasizes community integration, independence, and productivity, has daily implementation goals, and is monitored by county case management.
Shift: will be decided based on the position needed for hire.
Pay: starting wage is $16.00 per hour worked and can go up to $20 per hour worked for day and swing and can only go up to $19.00 for NOC Shift.
Section 2: Working Conditions
Must have the ability to pass and maintain the following:
- Oregon Intervention System Certification.
- CPR/First Aid Certification.
- Workday Oregon Core comp certifications (Tier1&Tier2)
- Medication Administration Skills.
Collaborative Living will train and provide these skills and certificates.
The work site is in a home environment, which is staffed 24 hours/day, seven days/week. Residential care is provided for adult supported residents. A group home may be “locked” or “unlocked” depending on the security needs of the supported individuals in the homes and the surrounding communities. Kitchens, laundry, and cleaning areas may be locked in some of the group homes based on the individual’s Individual Support Plan or Positive Behavior Support Plan. Furniture and wall decorations in the “locked” homes may be secured to the floors and walls for the safety of the individuals and the staff.
· The work hours and shifts of this position will meet the needs of the home and will be expected to work any shift, including weekends and holidays.
· Must be able to work occasional, required overtime to ensure adequate staffing requirements. · Must be able to move to different houses, within reasonable distance, to meet staffing requirements. · Work with combative, unpredictable, argumentative, aggressive adults who experience intellectual or developmental disability and who may be a danger to themselves and others daily.
· Frequent daily lifting up to 75 lbs. involving transferring, positioning, and moving individuals and/or moving program supplies. Frequent daily squatting, bending, and stooping.
· Must be able to engage and maintain Oregon Invention System (OIS) approved Protective Physical Interventions (PPIs) for one (1) hour or less.
· Daily exposure to household and sometimes industrial strength cleaning agents. · Exposure to a variety of work settings both within the home(s) and the community; This includes and is not limited to: exposure to activities of daily living that an individual may engage in such as, loud television noises or music, scented perfumes or air fresheners, foul language, discussions involving individuals’ preferences in religion or entertainment materials, posters, books, magazines and clothing that reflect individual preference.
· Occasional exposure to inclement weather.
· Must maintain a valid driver’s license and an acceptable driving record as frequent use of a motor vehicle to transport adults throughout the area to and from appointments and desired approved locations. · Dress in semi-professional work attire (examples: court, family visits, school, etc.).
· Must be able to speak, read, and write in English.
· Must be able to accompany supported individuals on hikes, walks, biking, swimming, and other physical activities.
· Must have basic computer skills / be able to utilize the THERAP system for documentation and communication with supervisors and co-workers.
· Skill in providing clear and concise verbal and written communications.
· Skill in prioritizing a heavy and often changing workload.
· Skill in working with individuals who may experience emotional, behavioral, or physical challenges. · Skill in resolving conflicts in a professional and timely manner.
Section 3: Job duties and responsibilities
60% Support the Individuals in our care by following their documentation • Follows supervision protocols for each individual both in the home and in community settings. • Daily support of individuals with I/DD who experience a co-occurring disorder and who may act aggressively towards themselves or others.
- Observes supported individuals for potential violence and socially destructive behavior and intervenes following agency and program policies and procedures to mitigate behaviors before they happen. • Spends time interacting with supported individuals to develop rapport in order to facilitate positive behavior support.
- Follows medical and safety protocols.
- Serves as a role model for supporting individuals in the group home and in the community. •
Administers medication and completes treatments following the medication administration curriculum. Documents daily and accurately on MAR, control medication count sheets, consultation forms, intake and output/bowel care records and all required med administration records. Treat injuries, oral and topical medications, transcribing physician orders and order medications.
- Teaches and demonstrates problem solving skills and anger management skills. • Responds to crisis situations at their group home, in the community or assists another CL home as needed. • Follows levels of supervision for general community integration, court appearances, school, family visits, medical appointments, counseling, and others as needed. • Follow Oregon Intervention System (OIS) of least to most restrictive strategies. • Participates and/or provides input in training and treatment plans. Provides objective feedback to behavior specialists and assists with interview/observations as requested by the team.
- Follows written ISP and PBSP and all support protocols and documents.
- Complete GERs as specified by policies and procedures related to behavior and other incidents. • Maintains continuous observation of individual or behavior/health needs and reports any changes/concerns immediately to the Site Manager followed by written documentation on a GER, T-Log and/or THERAP health tracking.
- Completes daily and accurate documentation on; Behavior data, training programs, outing requests, mileage logs, progress notes (T-Logs) etc.
- Monitors, assists, and motivates children or adults to participate in daily training and treatment needs directed by plan as well as incidental training.
- Monitors and assists with physical care, personal hygiene, dressing and prosthetic devices, etc. • Assists with transition upon individuals transfers within and exits from Collaborative Living. • Appropriately handles individuals’ money and returns/ documents receipts to the manager immediately upon return from an outing. Completes shift spending log EACH shift per policy. • As per an individual’s ISP/PBSP, may need to perform searches of individual’s environment for weapons or contraband; performs scheduled and random searches of supported individuals’ areas and facilities for contraband and weapons; uses personal protective intervention (PPI) procedures on supported individuals in crisis, as outlined in the individual’s PBSP. • Protects supported individuals from their peers who present a threat to their safety; makes regularly scheduled checks to account for the whereabouts of supported individuals.
- Monitors supported individuals on suicide precaution observation.
- Transport supported individuals during various daily activities such as doctors' appointments, court hearings, community outings and activities, and family visits.
10% Training and Communication
- Provides skills training for supported individuals living in the group home based on the Positive Behavior Support Plan (PBSP) or Individual Support Plan (ISP).
- Provides objective feedback to the treatment team and assists with interviews and observations as requested by the treatment team.
- Mentor new employees as assigned.
- Contributes to individuals' Personal Focus Worksheets.
- Attends and actively participates in job related training and implements as appropriate on the job, including but not limited to OIS, CPR, Med administration, individualized ISP/PBSP and revisions as they occur, annual CL training packet and Mandatory abuse training, collaborative problem solving.
- Participate in house, island, agency, meetings and attend required training as required or assigned. • Collaborates and communicates with coworkers and members of the individual’s ISP team to accomplish work and solve problems.
10% Household Duties
- Plans menus with input from supported individuals (including special diets and individual’s personal choices), orders and purchases food and household supplies; prepares meals for/with supported individuals. • Performs interior housekeeping duties such as laundry, and cleaning, dusting, emptying garbage, mopping floors, sweeping, disinfecting, and all other duties related to home safety and sanitation.
- Ensures appropriate ongoing cleanliness to meet infection control standards. • Performs or oversees/assists laundry activities.
- Performs or oversees/assists individuals in performing interior housekeeping minor exterior landscaping and minor repairs.
- Maintain a safe working environment, participate in safety checks, maintain vehicle cleanliness and functionality.
- Takes vehicles in for maintenance as scheduled.
- May execute fire and emergency procedures pertaining to evacuations,
life-threatening or medical emergencies, as required.
10% Documentation Requirements
- Complete daily documentation including but not limited to: T Logs, Medication Administration, PVOs, GERs, Behavior Data, Outing Requests, Health Tracking, Training Data, Outing Logs, and Mileage Logs. • Documents support individuals' responses to treatment and share information with the treatment team. • Participates in maintaining record of supported individual care and treatment; enters, retrieves, updates and stores information in computer systems; completes daily documentation including but not limited to medication administration, behavior data, outing requests and logs, health tracking, training data, and mileage logs. • Appropriately handles supported individual’s personal funds; enters information into supported individual account logs; records expenditures, and balances accounts as requested. • Keep records and maintain records of spending and company cards and balance the accounts as used or directed.
- Provides input for changes in treatment programs.
5% Medical Support Services
- Perform assigned Registered Nurse delegated medical tasks, transcribes physician orders. • Administers medications and documents accordingly prescribed, medications, special diets, and adaptive equipment.
- Orders medication for supported individuals based on physician’s orders; checks all medication upon delivery to assure accuracy according to physician’s orders; completes narcotics count sheets as needed.
- Conducts checks on medication documentation to assure completeness and accuracy; notifies site manager concerning irregularities or concerns as appropriate as well as complete a GER as warranted. • Treats injuries, administers topical medications, provides daily wound care. Ensures security of all medication and supplies; destroys outdated medication as appropriate.
- Monitors and/or assisted individuals in daily living activities such as bathing, feeding, grooming, and dressing; assists with lifting, moving, turning, and walking supported individuals.
- Provides medical care such as collecting specimens, measuring blood pressure, pulse, and respiration; administers oral medications, supports individuals while in the home and in the community. • Attends to support individuals’ basic physical, emotional, and social needs.
- Assure individuals have items needed for activities of daily living; personal items, such as soap, toothpaste, toothbrush, shampoo, etc.
- Assists individuals with observing birthdays and holidays as they request. • Participate in preferred activities with supported individuals such as hiking, biking, picnics, movies, games, concerts, and other events, interests, and projects.
