American Traveler is seeking an experienced RN Case Manager for an outpatient clinic setting in Alaska, requiring an active AK RN license and at least 1 year of experience.

Ambulatory outpatient clinic setting providing care across the continuum for individuals and families
-
13-week assignment with a Monday-Friday schedule, 0800-1700
-
Responsibilities include comprehensive patient assessments, care planning, medication administration, and clinical procedures
-
Care coordination with providers and interdisciplinary teams including referrals and follow-up services
-
Documentation completed in the electronic health record

May float to other departments or assignments as needed

Active, unrestricted RN license in Alaska (or another U.S. state with an Alaska license application submitted prior to start for direct hire; agency staff must hold an active AK license)

Minimum of 1 year of RN experience required

BLS certification required

Case Management certification preferred

Graduate of an accredited nursing school; Bachelor's degree preferred

High school diploma or equivalent required

Flu vaccination required

Fingerprint cards must be submitted and processed prior to start, with a minimum 6-week lead time

Provides culturally responsive, patient-centered nursing care using independent clinical judgment aligned with Alaska Board of Nursing scope of practice

Recognizes changes in patient condition and initiates appropriate interventions or escalations

Educates patients and families on care, medications, and discharge needs

Advocates for patients and assists with healthcare system navigation

Participates in quality and performance improvement activities and reports safety concerns per policy

Demonstrates professionalism, teamwork, and flexibility, including providing guidance to ancillary staff and supporting peer or student education
