LHCSA Experience Required

The ideal candidates must have homecare experience with LHCSA and be responsible for:
- Conducting admission assessments and biannual reassessments for patients.
- Establishing and providing a plan of care for the patient based on results of the nursing assessment visit, client needs, and physician orders.
- Providing education to patient/family and paraprofessional staff.
- Conducting as-needed visits to patients who require additional support.
- Communicating changes in patient’s condition, treatment, and meds directly to the agency.
Nurse Requirements:
- Graduate of an accredited school of nursing (RN or BSN).
- Current New York State RN license and registration.
- Home care experience with LHCSA required.
- Experience with NHTD, TBI, SOC, and UAS assessments preferred.
- Must have a vehicle to travel to patients
- Bi-Lingual preferred (but, not a requirement)
- Travel throughout the lower Hudson Valley Region

Compensation
- Competitive pay-per-service rates.
- Premium pay available for nurses with extensive home care experience and for those willing to travel throughout the lower Hudson Valley region.
- Per diem rate structure will be discussed during the interview process.
