Jelani Healthcare and Family Services is seeking a compassionate, organized, and dedicated Registered Nurse (RN) to provide nursing oversight and clinical support for adults with intellectual and developmental disabilities living in Community Integrated Living Arrangements (CILAs). The RN will ensure residents receive quality healthcare while maintaining compliance with Illinois Department of Human Services (IDHS) and Illinois Department of Public Health (IDPH).

Responsibilities
- Provide nursing oversight for individuals residing in CILA homes.
- Complete comprehensive nursing assessments and ongoing health evaluations.
- Develop, implement, and update Individual Health Care Plans (IHCPs).
- Coordinate medical care with physicians, specialists, pharmacies, hospitals, and other healthcare providers.
- Review physician orders and ensure implementation by Direct Support Professionals (DSPs).
- Oversee/train DSPs on medication administration, medication storage, and documentation.
- Train and educate DSPs on:
- Medication Administration
- First Aid/CPR
- Infection Prevention
- Health Monitoring
- Medical Procedures
- Emergency Response
- Monitor residents with chronic medical conditions and communicate changes to physicians and families/guardians.
- Conduct routine health monitoring including:
- Vital signs
- Weight
- Blood glucose monitoring
- Wound care
- Other nursing procedures as needed
- Participate in interdisciplinary team meetings.
- Maintain accurate nursing documentation and ensure compliance with state regulations.
- Assist with incident investigations involving medical concerns.
- Ensure compliance during IDHS and Medicaid surveys.
- Participate in quality assurance and risk management activities.
- Provide after-hours nursing consultation when assigned.
Qualifications Required
- Current Illinois Registered Nurse (RN) License in good standing
- Graduate of an accredited nursing program
- Minimum 1 year nursing experience
- Valid Illinois Driver's License
- Reliable transportation
- Ability to pass:
- Criminal Background Check
- Health Screening
- Drug Screening (if required)
- Current CPR/BLS Certification
Preferred
- Experience working with individuals with intellectual or developmental disabilities
- Previous CILA, ICF/DD, group home, behavioral health, or community nursing experience
- Knowledge of Illinois CILA regulations (Optional)
- Experience training care staff or direct care staff

- Leadership and staff coaching abilities
- Knowledge of electronic health records

Physical Requirements
- Ability to lift up to 50 pounds with assistance.
- Ability to stand, bend, walk, and assist individuals as needed.
- Ability to respond quickly during emergencies.
- Ability to travel between residentials
