Patient Volume: 1 Patient Per Hour

Conduct mental health assessments and develop individualized treatment plans.

Provide outpatient therapy to adults and/or children.

Deliver crisis intervention and behavioral stabilization as needed.

Maintain a structured caseload of one patient per hour to ensure quality care.

Work side-by-side with Primary Care Providers within our MindPATH model.

Collaborate with care managers and interdisciplinary teams.

Connect patients with appropriate community and behavioral health resources.

Maintain accurate treatment plans and progress notes in the EHR.

Monitor patient outcomes and adjust interventions as needed.

Prepare discharge planning and case summaries.

Master’s Degree in Social Work, Counseling, Psychology, or related field.

Active Illinois license in one of the following:

LCSW (Licensed Clinical Social Worker)

LCPC (Licensed Clinical Professional Counselor)

Minimum 3 years of experience providing outpatient behavioral health services.

Bilingual Spanish/English

Experience with assessments, treatment planning, therapy, and crisis intervention.

Ability to practice independently in the state of Illinois.

Experience in integrated or primary care behavioral health settings.

Comfortable collaborating with medical providers on medication management.

Bilingual (English/Spanish) strongly preferred.

Experience working with underserved or vulnerable populations.
