Community Support Services Specialist, Older Adults

This is an on-site, full-time role
Job Responsibilities:

Conduct intakes for our older adult programs and other JCC programs and services.

Perform outreach to synagogues, local organizations, physician offices, health care agencies, government agencies and community members.

Provide external referrals as appropriate.

Perform crisis intervention, short-term counseling and connect to concrete services.

Develop and maintain a resource guide for community referrals

Coordinate all aspects of assigned older adult program activities including transportation if needed

Ensure implementation of the agency's policies and practice

Provide psycho-social client intakes and Person-Centered Care Plans

Communication with clients, caretakers and families

Provide an environment of dignity and respect for all clients
- MSW or LMSW preferred with two to three years of experience working with Older Adults

Must be very familiar with Jewish practice and observance

Ability to work with diverse teams and clients.

Excellent verbal and written communication skills

Grant writing experience a plus
$45,000 - $55,000
