POSITION DESCRIPTION
Position Title: Forensic Monitor
Department: Connections Recovery Services
Reports To: Resource Coordinator

The Forensic Monitor will be a member of the Connections Recovery Services team. The Forensic Monitor is a specialized case manager who is responsible for tracking the progress of individuals deemed not guilty by a reason of insanity plea (NGRI), or found incompetent to stand trial and also deemed unrestorable. The forensic monitor works with individuals who meet these criteria through the court and works to create/ assist in the creation the conditional release plan. The Forensic Monitor then monitors treatment compliance outlined in the conditional release plan for the entirety of the plan. The Forensic Monitor acts as a liaison between the courts, psychiatric hospitals, probation departments, mental health providers and the client.
ESSENTIAL DUTIES AND RESPONSIBILIITES
This job description is not intended to be all-inclusive. Employee will also perform other reasonably related duties assigned by immediate supervisor.
- The Forensic Monitor ensures people with mental illness have a successful transition from the hospital to the community.
- The Forensic Monitor also ensures that effective risk management procedures are implemented by community providers and that prompt re-hospitalization, re-institutionalization, or detention occurs when public safety necessitates this level of care.
- The Forensic Monitor reviews patients' treatment plans, medication adherence, and progress in outpatient programs.
- The Forensic Monitor consults with community mental health agencies, psychiatrists, and caretakers regarding patient treatment and community functioning.
- The Forensic Monitor compiles and submits progress notes, compliance reports, and paperwork for conditional release extensions.
- The Forensic Monitor provides expert testimony in court or at revocation hearings regarding client status and progress.
- The Forensic Monitor attends meetings at NOBH, police departments or other mental health providers as needed.
- The Forensic Monitor provides court advocacy to clients involved in the criminal justice system.
- The Forensic Monitor completes all required charting, casework, documentation, and computer entry, on a timely basis.
- The Forensic Monitor attends and participates in all required training, staff meetings, and case conferences.
- The Forensic Monitor maintains a working knowledge of the mental health and human services systems in our region to provide appropriate referral and linkage for clients served.
- The Forensic Monitor performs tasks that provide for patients’ safety including ongoing screening and documentation of risk factors.
- The Forensic Monitor maintains a valid Ohio driver's license.
EDUCATION/EXPERIENCE/SKILLS
A Bachelor Degree in Criminal Justice, Social Work, Psychology or similarly related field.
Experience as a probation/parole officer preferred but not required.
Holds a valid Ohio driver’s license and passes a criminal background check.
SUPERVISORY RESPONSIBILITIES
This position supervises the following positions:

Required to respect the confidentiality of information concerning consumers who utilize Connections Recovery Services. Must maintain confidentiality according to HIPAA and 42 CFR Part 2 regulations.
