The Role
As RN Case Manager, you are the primary point of contact for your team of field clinicians — owning the full episode of care for your patients from admission through discharge. You'll deliver skilled nursing care, manage your caseload, and lead your team with coaching, mentorship, and clinical expertise.
Key Responsibilities
- Conduct comprehensive nursing assessments at admission, resumption of care, recertification, and discharge
- Develop, implement, and evaluate individualized Plans of Care with patients, families, physicians, and the IDT
- Deliver direct skilled nursing care including wound care, medication management, IV therapy, and patient/caregiver education
- Perform accurate and timely OASIS assessments for reimbursement, quality reporting, and patient outcomes
- Supervise and direct LPNs/LVNs, home health aides, and support staff
- Facilitate case conferences, approve orders, and perform utilization management review
- Monitor patient progress and adjust care plans to optimize outcomes
- Maintain accurate, timely documentation in the EMR in compliance with OASIS guidelines and regulatory requirements
- Manage a productive caseload with efficient visit scheduling within the assigned territory
- Participate in on-call rotations as required
What You Bring
- Active RN license in the state of agency operation
- Minimum 1 year RN experience; home health leadership background required
- OASIS experience required; HCHB experience strongly preferred
- Valid CPR certification, driver's license, and insured vehicle

Compensation & Benefits
$110,000 – $135,000 based on experience Medical, dental, and vision coverage ️ PTO and paid holidays 401(k) with employer match 2-week paid orientation Full-time | Monday – Friday | 8:00 AM – 5:00 PM
