Full-Time Registered Nurse (RN), Home Health

Center of Illinois Home Healthcare, Inc. is seeking a compassionate and experienced Full-Time Registered Nurse (RN) to join our Medicare-certified, ACHC-accredited home health team. The ideal candidate is dedicated to providing high-quality, patient-centered care in the home setting while ensuring compliance with Medicare regulations and agency standards.
Responsibilities

Provide skilled nursing visits and comprehensive patient assessments in the home.

Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.

Complete accurate and timely OASIS assessments and clinical documentation.

Educate patients, families, and caregivers on disease management, medications, treatments, and home safety.

Monitor patient progress and communicate changes in condition to physicians and other healthcare providers.

Coordinate care with physical therapists, occupational therapists, speech therapists, home health aides, and other members of the care team.

Ensure compliance with CMS, ACHC, Medicare, and agency policies and procedures.

Participate in case conferences and quality improvement initiatives.

Maintain accurate documentation using the agency's electronic medical record (EMR) system.
Qualifications

Active Illinois Registered Nurse (RN) license required.

Minimum of 2 years of home health nursing experience preferred.

OASIS experience preferred.

Strong knowledge of Medicare home health regulations.

Experience with Axxess EMR preferred.

Excellent assessment, organizational, and communication skills.

Ability to work independently while managing multiple patient visits.

Current CPR certification required.
