Duties
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance using proper manual handling and patient transfer methods such as Hoyer lifts when necessary
- Monitor patients’ vital signs, observe for changes in condition, and report findings accurately to healthcare professionals
- Support medication administration following HIPAA guidelines and care plans to ensure safety and compliance
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed
- Provide personal transportation assistance for appointments, errands, or community outings to promote independence and social engagement
- Maintain a clean, safe environment by performing light cleaning duties and ensuring infection control standards are met
- Document patient observations, care provided, and any incidents clearly and thoroughly in accordance with home health protocols
Requirements
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable
- Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, or working with individuals with neurodevelopmental or intellectual disabilities is preferred
- Ability to perform patient monitoring tasks such as vital signs measurement and patient observation with attention to detail
- Familiarity with care plans, medication administration procedures, CPR/First Aid certification, and HIPAA regulations is essential
- Experience working with individuals with autism or developmental disabilities is a plus
- Physical ability to lift heavy objects safely using proper manual handling techniques and assist with patient transfers using equipment like Hoyer lifts
- Strong communication skills to effectively interact with clients, families, and healthcare team members
- Compassionate attitude combined with a proactive approach to caregiving ensures high-quality support for residents’ physical and emotional needs
