Overview
Join our dedicated healthcare team as an On-Call Registered Nurse (RN) specializing in Home Health and Hospice services. In this vital role, you will provide compassionate, high-quality nursing care to patients in their homes, ensuring comfort, safety, and optimal health outcomes.
Duties
- Follow a 7-days-on, 7-days-off schedule, providing consistent hospice nursing support during assigned shifts.
- Serve as the primary responder for patient care needs during evenings, nights, weekends, and holidays.
- Triage urgent patient and family calls, delivering in-home nursing care when clinically indicated.
- Perform comprehensive patient assessments and implement timely, appropriate interventions.
- Administer medications and manage symptoms with clinical expertise, compassion, and attention to comfort.
- Provide education, guidance, and emotional support to patients and families during challenging and end-of-life situations.
- Collaborate closely with the interdisciplinary care team to ensure coordinated, high-quality patient care.
- Maintain accurate, detailed, and timely documentation in the electronic medical record (EMR) system.
Experience
- Current Utah RN license with strong clinical skills and sound nursing judgment.
- Able to remain calm, confident, and effective in high-pressure situations while working independently.
- Hospice or home health experience preferred, but not required; a passion for compassionate patient care is essential.
- Excellent communication and interpersonal skills, with a genuine commitment to serving patients and families.
- Flexible and dependable, with a willingness to participate in scheduled on-call rotations.
