Description:
Lake Park Center is a Specialized Mental Health Facility and is currently seeking a Qualified Mental Health Professional (QMHP) who is responsible for developing and providing recovery-based psychiatric services and case management to consumers with mental illness.
Qualified Mental Health Professional (QMHP) Responsibilities:

The QMHP administers assessment tools to incoming and current consumers.

Prepares written service plans and treatment update summaries, incorporating input from consumers, other service providers, wraparound team members to identify problem areas and needs, service objectives, and intervention strategies.

Provides individual and group counseling, therapeutic intervention, and/or substance abuse education classes to clients to assist them in recognizing problems and causal factors related to substance abuse and developing alternative/coping behaviors.

Utilizes Point Click Care (Electronic Medical Record System) and other web-based software for all clinical documentation, record keeping, and coordination.

The QMHP provides individual and group skills training and counseling services designed to improve consumer functioning and movement toward recovery.

The QMHP provides crisis intervention services or acts as the consultant to an MHP who is providing crisis intervention services.

Meets productivity requirements by utilizing various techniques to engage consumers and minimize failed appointments and maintaining an acceptable case load which engages and motivates clients throughout treatment.

Supervises the MHP’s, trains new team members, and provides guidance.

Attends court hearings and probationary meetings and presents testimony and treatment recommendations as requested by attorneys.

Prepares written Discharge Summary and Plan.

Attends bi-weekly staff meetings, group supervisions, and/or trainings.

Employee may be asked to perform other duties as required by the employer’s business needs.
Requirements:
Qualified Mental Health Professional (QMHP) Qualifications

Master’s or doctoral degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology or a related field and has successfully completed either: 1) practicum (or internship) that included a minimum of 1,000 hours of supervised direct service in a mental health setting; or 2) minimum one year of documented clinical experience under the supervision of a QMHP.

The QMHP must demonstrate significant independence in assessing psychiatric symptoms and consumer needs, teaching community living skills, directing, and providing crisis intervention, and making sound clinical decisions.

The QMHP understands the importance of building positive, therapeutic relationships with consumers to create an environment of trust and hope.

Knowledge and skills in the development and implementation of service plans.

Knowledge and skills in crisis intervention.

Knowledge of community resources.

Ability to maintain effective working relationships with consumers, families, and other service providers.

Ability to communicate effectively and professionally, both orally and in writing.

Desire and ability to work under supervision and cooperatively with a treatment team.

Must be at least 21 years of age.
