As a Blakeford Social Worker, you will:
- Counsel residents and their families on emotional, environmental or physical observations and interventions.
- Conduct regular face-to-face visits with assigned residents
- Facilitate active participation in care planning
- Serve as resident liaison for social, health, and community services that meet individual needs throughout discharge planning activities.
- Compile performance metrics and present summary data
