What You'll Do
As a Home Health Physical Therapist, you'll provide skilled therapy services to patients in their homes throughout the assigned territory. Responsibilities include:
- Perform evaluations and develop individualized plans of care
- Provide skilled physical therapy treatment in patients' homes
- Educate patients and caregivers on exercises, mobility, and safety
- Document visits accurately and within required timelines
- Collaborate with physicians, nursing, and other members of the care team
- Manage your daily patient schedule and assigned territory
What We're Looking For
- Active Colorado Physical Therapist license or ability to obtain one
- Graduate of an accredited Physical Therapy program
- Valid driver's license and reliable transportation
- Strong time-management and documentation skills
- Ability to work independently in a field-based environment
