We are looking to add a dedicated full time RN Case Manager to our wonderful Hospice team.
Primary functions are to administer nursing care for a 12 patient caseload in their place of residence, coordinate care with the interdisciplinary group, patient, family and referring agency; and assume the responsibility for coordination of care.

Hospice Case Manager Minimum Education & Experience Requirements:
- Current RN License – active and in good standing
- Minimum of 1-year nursing experience preferred
- Hospice experience is a plus
- Current CPR certification
- Strong organizational and communication skills
- Proof of current auto insurance
- Active Texas Driver's License

Pay: $80,000.00 - $90,000.00 per year
