Licensed Practical Nurse (LPN) / Registered Nurse (RN) – Home Health Care
We are seeking highly skilled and compassionate Licensed Practical Nurses (LPN) and Registered Nurses (RN) to join our growing healthcare team. Under the supervision of the head nurse, you will provide excellent care to patients in the comfort of their home, ensuring their safety, comfort, and overall well-being.
Key Responsibilities:
- Observe, monitor, and document patient conditions, including any changes or adverse reactions to medications or treatments, and take necessary actions.
- Measure and record vital signs such as height, weight, temperature, blood pressure, pulse, and respiration.
- Provide basic patient care, including temperature and blood pressure monitoring.
- Collaborate with the healthcare team to assess patient needs and implement appropriate interventions.
- Evaluate the outcomes of nursing interventions and consult with other healthcare professionals as necessary.
- Operate and manage medical equipment, including tracheostomy tubes and oxygen supplies.
- Provide tracheostomy care, including daily maintenance and respiratory assessments.
- Record food and fluid intake/output and assist with G-tube feeding and medication administration.
- Ensure patient comfort, safety, and well-being through attentive and compassionate care.
Required Skills/Abilities:
- General knowledge of nursing practices and healthcare terminology.
- Strong patient care skills with a compassionate, patient-centered approach.
- Ability to work independently in a home care environment with minimal supervision.
- Knowledge of tracheostomy care, respiratory assessments, and medication management.
Education and Experience:
- Postsecondary certificate from a vocational school or associate’s degree in nursing (required).
- Current state licensure as an LPN or RN (required).
- BLS Certification (required).
Physical Requirements:
- Ability to stand, move, and bend for extended periods.
- Must be able to lift up to 50 pounds when necessary.
