- Administer medications, including injections and IV infusions, following strict protocols and ensuring patient safety.
- Manage complex wound care, including dressing changes and infection control measures, especially in cases involving diabetic ulcers or post-surgical wounds.
- Provide specialized care for patients with chronic conditions such as Alzheimer's disease, dementia, or long-term care needs in assisted living or skilled nursing facilities.
- Educate patients and their families on disease management, medication administration, nutrition, and safety practices to promote health literacy.
- Document all patient interactions accurately using Electronic Health Records (EHR) systems like Epic or Cerner while maintaining compliance with HIPAA and confidentiality policies.
- Collaborate with physicians, case managers, and other healthcare professionals to coordinate care plans, discharge planning, and follow-up services.
Experience
- Proven experience working with diverse patient populations in home health settings or similar environments such as hospitals, nursing homes, or outpatient clinics.
- Critical care experience including ICU or emergency medicine is highly valued for managing acute situations effectively.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks for accurate medical documentation.
- Knowledge of ICD coding (ICD-9/10), CPT coding, and medical terminology to ensure precise recordkeeping and billing processes.
- Experience working with individuals with disabilities or developmental challenges enhances your ability to deliver empathetic and effective care.
- Familiarity with long-term care regulations, hospice care principles, and dementia/Alzheimer’s care best practices ensures comprehensive support for complex cases.
- Skilled in performing venipuncture, specimen collection/processing, catheterization, sterile techniques, and basic life support (BLS/CPR).
- Ability to adapt quickly to various clinical environments such as urgent care, outpatient clinics, or home settings while maintaining high standards of safety and professionalism.
