Responsibilities:
· Perform personal care needs of patients in accordance with the plan of care.
· Assist with personal hygiene including baths, oral hygiene, shampoo and changing bed linens as often as assigned.
· Provide nutritional support, feeds patients and prepares snacks as assigned.
· Assist in dressing and undressing patients as assigned.
· Provide respite for patient/family/caregivers when on-site, as appropriate.
· Keep patient’s living area clean and orderly as assigned.
· Adhere to the organization’s documentation and care procedures and standards of personal and professional conduct.
· Attend team meetings as requested and maintains effective communication with other team members.
· Perform other job-related tasks as requested.
Qualifications:
· Meets the training requirements of Georgia.
· Has ability to read and follow written instructions and document care given to patients.
· Must Have a reliable transportation with a valid driver’s license
· Is self-directed with the ability to work with little direct supervision.
· Provides a calm manner when in a patient home.
· Has empathy for the needs of the ill, injured, frail and the impaired.
· Is flexible and cooperative in fulfilling role obligation.
· Demonstrates tact, patience and good personal hygiene.
· Good oral and written communication skills.
· Good organizational skills.
