Working hours: PM shift (1:45pm-10:15pm) working every 3rd weekend
- Assure allegations of abuse or any other incident required by the WRC Abuse and Incident Management Policy is reported immediately and immediate steps to ensure that an individual involved receives appropriate treatment and protection from further harm.
- All assessments will be completed according to WRC protocol and procedure per assignment-this may include quarterly, MOSES, DISCUS, areas of risk, etc. Nursing care will be evaluated and documented according to protocol and procedure. This may include monthly reviews, annual ISP reports, etc.
- Health care problems for Individuals (including, but not limited to completion of incident reports, alleged abuse reports, temporary health care problems will be assessed, referred to the appropriate discipline, and documented in the Interdisciplinary Program Record (IPR) using SOAP (Subjective-Objective-Assessment-Plan)method of charting with follow up according to protocol.Physical assessments are performed and documented in IPR by the end of the shift on assigned individuals. Individuals who return from the Medical Center to the house will have assessments documented according to protocol.
- Illness and injury follow up and reassessment is completed according to nursing protocol.
- Use universal precautions 100% of the time for all anticipated contact with blood and body fluids. All potential exposures to blood and body fluids is reported according to procedures.
- Complete medication administration monitoring according to PNM guidelines.
- Monitor restraint use and maintain the Individual's safety by completing required assessments for each restraint use according to facility policy.
- Prioritize daily work assignments according to the severity of the Individual's conditions.
- Communicate professionally as a member of the Interdisciplinary Team.
- Administer medications, enteral feedings, nebulizer treatments as assigned.
- Health care provider orders will be entered in the electronic order entry system, and will be timely and accurate. Orders will be implemented according to policy and procedure Medication Administration Records will be complete and accurate. Medication orders will be transcribed accurately according to policy and procedure.
- Medication and other order changes will be discussed and communicated to the IDT. The 24 hour book and nursing report books will be updated, and appropriate follow of health care concerns completed
- Provide ongoing training of direct care staff and other members of the IDT.
- Provide med aide candidates with clinical experience through training and supervision in administering medications.
- Provide quarterly monitoring of medication aides. Ensure medications are administered safely, and according to WRC procedures.
- Provide competency based training to direct care staff and other members of the IDT on Health and Safety Support Plans.
- Successfully complete annual nursing skill training, and remain certified in Health Care Provider CPR.
- Provide appropriate follow up of all health care concerns by documenting on the 24-hour book and in the nursing report.
- Complete information for all off campus facilities, and home visits according to WRC policy and procedures.
- Perform additional duties as assigned.
Mandatory requirements:
- Licensure as Registered Nurse in the State of Iowa by the Iowa Board of Nursing
- Applicant must be able to pass a level 3 background check as a condition of employment.
Essential functions (one per line):
- Interact with Individuals with developmental disabilities and provide direct care as required.
- Regular and punctual attendance at the assigned work site based on work schedule assigned; work overtime or other
- shifts and/or work assignments to provide coverage for the facility when directed by management.
- Provide nursing care, and to plan, implement, and evaluate health care provided to Individuals served.
- Complete quarterly physical examination of Individuals and assessment of medication side effects.
- Document physical assessments of all reported injuries and illness and determine the appropriate intervention.
- Provide emergency first aid, CPR, administration of oxygen, and oral suctioning.
- Administer medications/treatments correctly and accurately.
- Accurately transcribe and note medication orders.
- Successfully complete all required training.
- Communicate effectively with physicians, Individuals, public, and other team members.
