Overview
Independent Primary Care office is hiring for a Clinical Nurse LPN to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience in various clinical settings. As a Clinical Nurse, you will be responsible for providing high-quality nursing care to patients, ensuring their comfort and well-being while adhering to established protocols and standards. Your expertise in areas such as critical care, geriatrics, and acute pain management will be invaluable in delivering exceptional care.
Responsibilities
- Provide comprehensive nursing assessments and develop individualized care plans for patients.
- Monitor patient vital signs and via RPM reporting any significant changes to the healthcare team.
- Assist with procedures such as injections, catheterization, wound and skin care when necessary.
- Collaborate with interdisciplinary teams to coordinate patient care with Home Health, Palliative and Hospice patient's .
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Educate patients and their families on health management, medication administration, and post-discharge care.
- Participate in quality improvement initiatives and adhere to NCQA standards.
- Provide support for patients with developmental disabilities, memory care needs, or chronic conditions.
Qualifications
- Valid nursing license ( LPN) in the state of practice.
- Experience in acute care settings such as hospitals or long-term care facilities is preferred.
- Familiarity with EMR/EHR systems such as Epic or Cerner is advantageous.
- Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques.
- Proficiency in performing basic life support (BLS) and advanced cardiac life support (ACLS) procedures.
- Strong communication skills for effective patient interaction, education and collaboration with healthcare professionals.

Pay: $25.00 - $30.00 per hour
