Position: Full-Time | Hospice Care
Shift: Day Shift
Contract: 12 Weeks
Service Area: White River Junction, VT & West Lebanon, NH
Pay Rate: $45/Hourly
Mandatory Qualifications for this Job
- Active, unrestricted Vermont or New Hampshire LPN License or Compact LPN License
- Graduate of an approved Practical Nursing Program
- Experience in Medical-Surgical Nursing and/or Home Health Care
- Strong patient assessment and clinical care skills
- Ability to provide compassionate end-of-life and hospice care
Roles & Responsibilities
- Provide direct hospice care during home visits
- Assess patients and monitor condition changes
- Perform nursing interventions under RN supervision
- Support end-of-life and comfort care
- Educate patients and families on care plans
- Document and report patient status changes
- Collaborate with RN Case Managers and care teams
- Promote patient comfort, dignity, and quality of life
- Maintain accurate clinical documentation
- Follow hospice, nursing, and safety standards
- Deliver compassionate, patient-centered care
