We are seeking Pediatric Coaches to work in the Cumberland and surrounding counties to assist children with medically complex diagnoses by providing all caregiver teams with non-medical education and training.
The Pediatric Coach will be working remotely but traveling in Cumberland, Perry, Dauphin, Lancaster, Lebanon, Franklin, Fulton, York, and Adams Counties.
Requirements:
- Supporting all members of caregiver teams to function successfully within the parameters of their roles and responsibilities;
- Documenting all work performed, including progress notes, caregiver planning, training utilization and other collateral documentation;
- Scheduling and facilitating Family Advisory Workgroup meetings;
- Collaborate with coworkers and Family Facilitators; and
- Meeting regularly with supervisors to report case summaries and all critical matters.
Essential Skills:
- Excellent communication skills;
- Ability to generate detailed reports;
- Ability to meet strict timelines;
- Flexible schedule; and
- Ability to work independently and as a team member.
Education/Professional Requirements:
- Bachelor’s degree in social work or related field; and
- Five years’ experience in the human services field.
