Job Type: Travel Contract (13 Weeks)
Schedule: Monday–Friday, 8:30 AM – 5:00 PM, with alternating weekends
Guaranteed Hours: 40 hours per week
Job Summary
We are seeking an experienced Home Health Registered Nurse (RN) for a 13-week travel assignment in Greenwich, Connecticut. The ideal candidate will have recent Home Health experience, strong clinical skills, and experience with OASIS documentation. Responsibilities
- Provide skilled nursing care to patients in their homes.
- Complete patient assessments and develop individualized care plans.
- Perform OASIS documentation accurately and efficiently.
- Coordinate patient care with physicians and interdisciplinary healthcare teams.
- Educate patients and families on disease management and treatment plans.
- Maintain timely and accurate clinical documentation.
- Participate in the on-call rotation as required.
Requirements
- Active RN license (must be in hand at the time of submission).
- Minimum 2 years of recent Home Health RN experience.
- Recent Homecare Homebase (HCHB) and OASIS experience within the last year is required.
- Strong assessment, documentation, and time management skills.
- Excellent communication and customer service skills.
- Reliable, professional, compassionate, and dependable.
Preferred Qualifications
- Experience with WellSky EMR.
- Wound care experience.
- Experience with on-call triage.
On-Call Expectations
- On-call rotation of 1–2 weeks per month.
- Primarily phone triage; home visits are rarely required.
Productivity Expectations
Full-time nurses are expected to complete 30 productivity points per week.
Point System:
- Revisit = 1 point
- Start of Care (SOC) = 2 points
- Resumption of Care (ROC) = 2 points
- Recertification = 1.5 points
- OASIS Discharge = 1.5 points
Pay: $56.00 - $61.00 per hour
