Responsibilities
- Provide attentive patient observation to monitor health status and respond promptly to changes.
- Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility assistance using tools like Hoyer lifts.
- Administer medications and perform basic medical procedures including catheter care, vital signs measurement, and hospice care support.
- Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining strict compliance with HIPAA regulations.
- Support clients with memory care needs by implementing dementia and Alzheimer's care strategies to ensure safety and comfort.
- Assist with meal preparation, feeding, laundry, cleaning, and other home care tasks to promote a safe and clean environment.
- Collaborate effectively with nursing staff and other healthcare professionals to ensure comprehensive patient care.
Skills
- Strong patient observation skills combined with experience in assisted living or long-term care environments.
- Knowledge of medical terminology, medication administration protocols, and vital signs monitoring.
- Proficiency in working with individuals with developmental disabilities or dementia-related conditions.
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping.
- Ability to perform CPR and handle emergency situations confidently.
- Experience in hospice or home & community care settings is highly desirable.
