Job Summary:
The Home Care Service Nurse is a permanent, full-time position with the Department of Public Health. This is professional nursing work in a specialized area within the home care program, providing home health services to adults and children in the counties covered by the West Central District of ADPH. Counties primarily served by the Tuscaloosa office are: Tuscaloosa, Bibb, Green, Hale, Perry, and Sumter. Counties primarily served by the Vernon office are: Fayette, Lamar, Pickens, and Walker.
Responsibilities:
- Completes comprehensive nursing assessment in the home utilizing the OASIS and other appropriate tools for home care.
- Provides skilled nursing care to patients in their home.
- Provides teaching to patients and caregivers on disease process, medications, wound care, self care as ordered by a physician or other medical provider.
- Participate in on-call rotation.
Qualifications:
- An Alabama Registered Nurse License is required.
- Associate's Degree or Diploma Nursing Degree and two years of professional nursing OR
- Bachelor's Degree in Nursing

Pay: $47,839.20 - $84,280.80 per year
