Position Summary
We are seeking a professional, dependable, personable, and highly organized Community Client Liaison to join our healthcare team.
The Community Client Liaison will serve as a key representative of the agency and will often be one of the first faces our clients and families meet after choosing our services. This position will travel to client homes to complete new-client onboarding, assist with required paperwork, introduce clients and families to our agency and services, conduct client touch points, and perform quality assurance reviews of new-client files.
The ideal candidate will have previous experience in Medicaid, Medicare, home health, home care, personal care services, healthcare administration, case management, or a related healthcare field. The candidate should have a basic understanding of healthcare documentation, client services, Medicaid/Medicare terminology, and the importance of maintaining accurate and compliant client records.
This position is ideal for someone who enjoys working directly with people, is comfortable traveling throughout the community, communicates professionally, and can confidently represent the agency.
Essential Duties & Responsibilities
Client Onboarding
- Travel to clients’ homes to conduct initial onboarding visits.
- Welcome new clients and families and provide a professional introduction to the agency.
- Assist clients with completing required onboarding, enrollment, and agency paperwork.
- Review paperwork with clients to ensure information is complete and accurate.
- Explain agency policies, procedures, expectations, and available services.
- Ensure clients understand how to contact the office and who to contact with questions or concerns.
- Identify missing information or documentation and communicate outstanding items to the appropriate office staff.
- Maintain professional and respectful relationships with clients and their families.
Client Engagement & Agency Representation
- Serve as a positive representative and “face” of the agency in the community.
- Build trust and establish strong relationships with clients and families.
- Clearly communicate the agency’s services, values, expectations, and benefits.
- Help prospective and newly admitted clients understand the services the agency provides.
- Present the agency in a professional, knowledgeable, and welcoming manner.
- Identify opportunities to improve the client’s experience and communicate recommendations to management.
- Maintain a professional appearance and demeanor during all client and community interactions.
Client Touch Points
- Conduct scheduled client touch points following admission.
- Contact or visit clients to ensure they are satisfied with their services.
- Confirm that clients understand their services and know how to contact the agency.
- Identify concerns regarding caregivers, scheduling, communication, or service delivery.
- Report client concerns or complaints to the appropriate supervisor or management team.
- Document all client interactions and follow-up activities accurately.
Quality Assurance & File Audits
- Conduct quality assurance reviews of new-client files.
- Review files for missing, incomplete, or inconsistent documentation.
- Identify onboarding deficiencies and communicate findings to the appropriate office staff.
- Assist with maintaining organized and complete client records.
- Follow up on outstanding documentation when directed.
- Assist management with quality assurance activities involving new clients.
- Maintain confidentiality when reviewing client records and sensitive information.
- Follow agency policies and applicable HIPAA and privacy requirements.
Healthcare & Compliance Responsibilities
- Apply knowledge of Medicaid, Medicare, home health, home care, and healthcare processes when performing assigned duties.
- Understand basic healthcare terminology and documentation requirements.
- Recognize when documentation or client information may require additional review by clinical or administrative staff.
- Communicate important client information to the appropriate member of the healthcare team.
- Follow agency policies, procedures, and compliance requirements at all times.
- Never provide clinical advice or make clinical decisions outside the scope of the position.
Required Qualifications
- Previous experience in healthcare, home health, home care, Medicaid, Medicare, healthcare administration, case management, or a related field preferred.
- Prior knowledge of Medicaid and/or Medicare healthcare services strongly preferred.
- Knowledge of home health or personal care services is highly preferred.
- Strong verbal and written communication skills.
- Excellent customer service and interpersonal skills.
- Professional appearance and demeanor.
- Strong attention to detail.
- Ability to review documents and identify missing or incomplete information.
- Ability to work independently in the field while maintaining communication with the office.
- Reliable transportation and a valid driver’s license.
- Ability and willingness to travel to multiple client homes.
- Comfortable using a smartphone, email, electronic documentation systems, and other basic technology.
- Ability to maintain confidentiality and handle protected client information appropriately.
Preferred Experience
Preference will be given to candidates with experience in:
- Home health agencies
- Non-medical home care
- Medicaid waiver services
- Medicare home health
- Personal care services
- Healthcare case management
- Client intake and admissions
- Healthcare documentation

- Electronic medical records (EMR)
- Prior authorization or Medicaid service coordination
- Client relations or healthcare outreach
Schedule & Availability
This is a part-time position requiring approximately three days per week.
The selected candidate must:
- Be available to work every other weekend.
- Be available for weekday client visits as scheduled.
- Be flexible based on client onboarding and agency needs.
- Be willing to travel between multiple client locations during scheduled workdays.
- Maintain reliable attendance and punctuality.
Compensation
Starting Pay: $17.00 per hour
Mileage Reimbursement: Mileage reimbursement will be provided for approved business-related travel in accordance with agency policy.
