General Overview:
We are seeking a dedicated and compassionate Case Manager to join our Health Homes Program in New York City. The Case Manager will play a critical role in coordinating comprehensive care services for individuals with complex health needs, including chronic conditions, mental health challenges, and substance use disorders.
Responsibilities:
- Assessment and Care Planning: Conduct comprehensive assessments of clients' medical, behavioral, and social needs to develop individualized care plans.
- Coordination of Care: Collaborate with healthcare providers, social service agencies, and community resources to ensure seamless coordination of care and services.
- Client Advocacy: Serve as an advocate for clients to navigate the healthcare system, access needed services, and overcome barriers to care.
- Monitoring and Evaluation: Regularly monitor clients' progress and outcomes, adjusting care plans as needed to achieve health and wellness goals.
- Crisis Intervention: Provide crisis intervention and support to clients experiencing acute medical or psychosocial crises.
- Documentation and Reporting: Maintain accurate and up-to-date documentation of client interactions, assessments, and care plans in accordance with program requirements.
- Team Collaboration: Collaborate with interdisciplinary team members, including nurses, social workers, and care coordinators, to ensure integrated and holistic care delivery.
- Community Engagement: Participate in community outreach and engagement activities to raise awareness of available services and resources.
Qualifications:
- Bachelor's or Master's degree in social work preferred, nursing, psychology, or a related field.
- Previous experience in case management, care coordination, or a related healthcare role preferred.
- Knowledge of healthcare systems, including Medicaid, Medicare, and managed care organizations.
- Strong communication, interpersonal, and organizational skills.
- Ability to work effectively in a fast-paced and dynamic environment.
- Bilingual proficiency in English and Spanish preferred (or other relevant languages).
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
