Responsibilities:
- Administer medications and monitor patient health and vital signs.
- Develop and implement individualized care plans for each client.
- Provide emotional support and education to clients and their families.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Document patient progress and maintain accurate medical records.
- Assist with daily living activities and promote client independence.
- Conduct health assessments and manage chronic conditions.
- Participate in training and development programs to enhance skills.
Requirements:
- Current Registered Nurse (RN) license in Virginia.
- Minimum of 2 years of nursing experience, preferably in a healthcare setting.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
- Compassionate demeanor with a focus on patient-centered care.
- Proficient in electronic health record (EHR) systems.
- CPR and First Aid certification required.
- Availability to work flexible hours, including weekends.
