Hegira Health's Children's Outpatient Services clinic offers quality, comprehensive, compassionate specialty services to children with serious emotional disorders and their families from a team of dedicated professionals.
Position requires flexible hours meeting with families after school or school, attend court and school meetings in the mornings. Wrap Facilitators flex and can start at 11:00 AM-12:00 PM and work until 7-8:00 PM.

REQUIREMENTS:
- A Bachelor's Degree from an accredited college or university, specializing in psychology, social work, counseling, or other related human services field
- Direct treatment services to children/adolescents in a behavioral health inpatient or outpatient setting for 1 year or more
- Experience in Wraparound or IDD/autism
- Demonstrate basic knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to the intellectually & developmentally disabled (IDD) child/adolescent population
RESPONSIBILITIES (summary):
- Provide services at frequency and intensity consistent with individual patient needs, but no less than one face-to-face contact per month.
- Completion of the Case Management Assessment.
- Makes recommendations for ongoing targeted case management services/supports coordination, including linking, coordination, advocacy, negotiation and referral services.
- Identify issues that may exist regarding safety (personal and environmental), natural supports, and community involvement.
- Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV risk, education, and vocational training.
- Coordinate appointments with psychiatrist and primary therapists.
- Provide linking and coordination of community-based services.
- Educate patient and patient parent/guardian with respect to medication assistance, MI Child, and Medicaid application processes, as necessary.
- Complete individualized Treatment Plan Case Management goals and objectives for each patient.
- Establish effective therapeutic bonds with patients assigned to caseload.
- Document termination of case management services and notify appropriate parties.
- Complete a Continuing Care Plan for each patient discharge.
- Document all patient contacts and/or contacts related to case management services.
- Assist patient and/or primary therapist with various case management/ supports coordination tasks, such as medication compliance, referrals, community resources.
- Assist with problem solving and behavior management concerns in regards to patient maladaptive behaviors or decompensation.
- Complete required annual documentation updates and/or referrals.
WRAPAROUND SPECIFIC
- Complete strengths-based discovery assessment on all clients/families.
- Prioritize child and family needs based on life domains.
- Identify cultural issues regarding the values, preferences, beliefs, culture, and identity of child and family and their community.
- Evaluate and assess individual strengths on the child and family teams.
- Complete individualized Wraparound Service Plan for each family.
- Develop services to facilitate Wraparound Plan and work with referring agency staff and Community Team in coordinating services.
- Using a Person-Centered Planning approach, customize set of strategies, supports, and services to meet the unique needs of each child and family.
- Tie goals and strategies of the Wraparound Plan to observable or measurable indicators of success, monitor and revise.
- Build on and enhance the capabilities, knowledge, skills, and assets of the child and family.
- Coordinate individual services as need.
