Job Summary
The Community Health Worker (CHW) provides quality bilingual, bicultural health education and screenings, advocacy, referrals and assistance to individuals and families served by St. Mary’s Health Clinics (SMHC). The CHW Works collaboratively in clinic and outreach settings to provide resources and support for patients, families and staff, including interpretive services as needed. Following clinic visits, the RN and CHW collaborate to provide essential patient care coordination such as documentation, referral appointments, data entry, etc. Outreach activities related to health promotion and wellness are functions of this position also, and requires a CHW presence at various community events.
Responsibilities
- Provide health information through one-on-one educational interventions as well as group presentations at clinics and community sites.
- Under supervision, perform blood pressure, diabetes or other health screenings as required at clinic sites.
- Assist patients and program participants with the enrollment for potential health insurance or pharmacy discount programs.
- Provide necessary advice to support individuals’ decision-making regarding choice of a medical home.
- Assist with the referral process for specialty care or community resources that may be available to promote optimal health.
- Document pertinent patient or program participant data in the patient medical record or outreach databases.
- Enter data in the SMHC database for recordkeeping, statistical purposes and financial stability.
- Attend health fairs and community events representing SMHC outreach, providing services as needed.
- Promote knowledge and cultural understanding of the population served to SMHC team members and community partners.
- Submit progress reports to nursing supervisors regarding clinic sessions, patient issues and concerns regarding general clinic operations
- Submit outreach reports.
- Participate in staff meetings, special projects and staff development sessions as required.
- Support activities of the Mexican and Ecuadorian Consulates as needed.
- Other responsibilities as assigned
Requirements
- High school diploma required.
- Fluent in Spanish with excellent verbal and written communication skills; proficiency in English is preferred.
- Valid Minnesota driver’s license and vehicle to allow for travel to various clinic/outreach sites.
- Healthcare experience, such as in a clinic or hospital, required.
- Strong knowledge and understanding of Latino / Hispanic culture required.
- Completion of CNA or Community Health Worker training and certification preferred. Would consider applicant meeting requirements with a commitment to complete CHW certification within a mutually agreed upon timeframe.

Pay: $25.00 - $28.00 per hour
