Care For the Homeless (CFH)* is a growing not-for-profit health care and social service agency that serves the homeless population of New York City. CFH operates multiple service delivery sites in four boroughs, including Article 28 licensed health centers that provide medical and behavioral health services that serve individuals experiencing homelessness throughout four NYC boroughs. CFH also operates two homeless women’s residences where health care services are provided.

This employee will work as part of the PCMH team with the following essential duties and responsibilities:*
Acts as clinical lead in morning huddles with clinical care and primary administrative coordination responsibility at assigned site.
Collaborate with PCP in the disease management model addressing condition-specific patient education needs
Implement orders for specific interventions as indicated by the medical staff
Support primary care provider for chronic or episodic care;
Participation in all prevention and treatment efforts such as Narcan provision to all patients with history of substance abuse
Provide clinical education for patients
Work collaboratively with care management and population health education staff to effectively coordinate care as directed by the primary care medical staff
Reach out and engage patients with respect to their health status, substance abuse and social service problems;
Advise and educate clients about their physical and mental health, self-care, prevention strategies
Take a complete history of individual and family members;
Assess the physical and psychosocial needs of the patient;
Administer immunizations, and document patient chart in EHR and CIR
Administer point of care testing (e.g. HIV, Hemoglobin A1C, pregnancy tests, and EKGs) and other onsite diagnostic procedures, ensuring proper documentation including billing
Triage patients, including medical, social, psychiatric, OBGYN, and surgical histories and complete medication reconciliation
Triage walk-ins to determine dame-day appointment needs
Dispense 340B medication and ensure proper tracking and logging
Immediate response in all emergency codes
Maintain medication and medical supplies to ensure proper disposal of expired items
Coordinate proper pick-up of emergency and hazardous items
Maintain all appropriate refrigerator and freezer logs for vaccines and report any excursions
Weekly inventory of all medical supplies, including vaccines
Maintain all licenses, certifications, and annual training requirements
Work in nontraditional environments, e.g., non-clinic settings.
Be able to travel across multiple sites (Bronx, Brooklyn, Queens, and Manhattan, Staten Island) as needed
Be able to work weekends based on organizational need for staff coverage
Required Education, Licenses, and/or Certifications:
- Minimum five (5) years experience in an ambulatory care setting, emergency room, and/or Physician office.
- Current NYS Registered Nurse licensure, Phlebotomy Certification, Basic Life Support (BLS) Certification, Electrocardiogram Certification
- Commitment to the poor and disadvantaged.
Preferred Education & Experience:
- Bachelor of Science in Nursing (BSN) preferred
- Prior eClinicalWorks (eCW) experience
