POSITION SUMMARY
We are seeking a compassionate and dedicated Hospice Medical Social Worker (MSW) to join our interdisciplinary team covering Contra Costa County on a part-time basis. This position includes patient and family home visits throughout Contra Costa County, psychosocial assessment and counseling responsibilities. The MSW plays a vital role in helping patients and families navigate the emotional, social, and practical challenges of end-of-life care, with a flexible schedule built around patient needs and visit volume.
RESPONSIBILITIES
Clinical / Field:
- Conduct comprehensive psychosocial assessments for patients and families in their homes throughout Contra Costa County
- Provide counseling and emotional support to patients and families coping with terminal illness, grief, and loss
- Develop and implement psychosocial care plans as part of the interdisciplinary team
- Assist patients and families with advance care planning, including advance directives and POLST forms
- Connect patients and families with community resources, financial assistance, and government benefits (Medi-Cal, Medicare, SSI, VA benefits, etc.)
- Provide crisis intervention and short-term counseling as needed
- Support bereavement services for families, including bereavement risk assessment and follow-up
- Collaborate with physicians, RNs, chaplains, home health aides, and volunteers to ensure holistic, coordinated care
- Maintain timely and accurate documentation in the EMR system
- Report changes in patient/family needs to the supervising RN Case Manager or Director of Patient Care Services
- Available for on-call or as-needed psychosocial support coverage, as scheduled
Quality Assurance:
- Ensure psychosocial documentation meets hospice regulatory and Medicare Conditions of Participation standards
- Assist with chart audits and corrections as needed
- Support survey readiness and ongoing compliance efforts, including Joint Commission and state requirements
- Participate in QAPI (Quality Assessment and Performance Improvement) activities as scheduled
Office / Administrative (Pleasant Hill, CA):
- Participate in IDG (Interdisciplinary Group) meetings as scheduled
- Coordinate with office staff and care coordinators on psychosocial and community resource matters
QUALIFICATIONS
- Master's degree in Social Work (MSW) from an accredited program (required)
- Active California Board of Behavioral Sciences registration or licensure (LCSW, ASW, or eligible) preferred
- At least 1 year of clinical experience; hospice, palliative care, or healthcare social work experience strongly preferred
- Reliable vehicle and valid driver's license required — must be able to travel throughout Contra Costa County
- Strong communication, organization, and critical-thinking skills
- Comfortable managing a flexible, visit-based caseload within Contra Costa County
- Compassionate, calm, and steady presence with patients and families during emotionally difficult times
- Knowledge of community resources, government benefits, and end-of-life planning a plus
- Familiarity with hospice documentation and Medicare hospice benefit guidelines a plus

OFFICE LOCATION
Pleasant Hill, CA — home base for IDG meetings.
SCHEDULE
- Per diem / part-time — flexible hours based on patient census and visit needs
- Field visits scheduled throughout Contra Costa County
- Availability for some weekday, weekend, or on-call coverage preferred
- Schedule built around your availability and patient needs
