Per-Diem Licensed Practical Nurse (LPN) – All Shifts!

Silver Stream Rehabilitation and Nursing Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team.
Responsibilities:
Administer medications and treatments as prescribed
Monitor residents' conditions and report changes to the RN Supervisor and physicians
Provide direct nursing care in accordance with facility policies and standards of practice
Accurately document resident care and services provided
Supervise and support Certified Nursing Assistants (CNAs)
Participate in care planning and interdisciplinary team meetings
Ensure compliance with all state, federal, and facility regulations
Qualifications:
Current LPN license in the Commonwealth of Pennsylvania
Long-term care experience preferred, but not required
Strong clinical, communication, and organizational skills
Ability to work effectively as part of a team
