Overview
Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) working a 7:00am to 7:00pm rotation. In this vital role, you will provide compassionate, high-quality nursing care to diverse patient populations across various settings, including long-term care, assisted living, and acute care environments. Your expertise will help ensure patient safety, comfort, and well-being while supporting interdisciplinary teams in delivering exceptional care. Responsibilities
- Deliver comprehensive nursing care to patients, including medication administration, vital signs monitoring, and physical assessments.
- Assist with patient intake, discharge planning, and documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Perform specialized procedures such as IV infusion, catheterization, tube feeding, and sterile dressing changes following aseptic techniques.
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans for patients with complex needs such as dementia, Alzheimer's disease, or traumatic injuries.
- Conduct triage assessments for urgent or emergent situations, ensuring timely intervention and appropriate escalation of care.
- Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while managing medical records and documentation.
- Support infection control protocols and participate in quality improvement initiatives to enhance patient safety standards.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
- Demonstrated experience working with diverse patient populations including geriatrics, pediatrics, or individuals with disabilities.
- Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience with medical documentation is preferred.
- Knowledge of nursing practices related to acute pain management, hospice care, trauma centers (Level I/II), or ICU settings is highly desirable.
- Strong understanding of medical terminology, anatomy, physiology, and diagnostic evaluation procedures.
- Ability to perform physical examinations and vital signs assessment accurately.
- Excellent communication skills for caregiver communication and client interaction; compassionate approach to patient care is essential.
