Community Healthcare of Texas is currently recruiting for an Inpatient RN.
POSITION SUMMARY
IPU RN/Case Manager assesses patients' physical, psychosocial, and spiritual condition and determines and/or revises the plan of care; provides palliative nursing care.
ESSENTIAL FUNCTIONS

Complete an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es)

Provide emotional support for patients and families/caregivers while instructing in end-of-life care/issues

Assess and evaluate patient’s status by writing and initiating plan of care. Regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary

Develop a care plan that establishes goals, based on nursing diagnosis, and incorporate hospice nursing actions. Include the patient and the family in the planning process

Case manage total plan of care of patient/support persons by assessing conditions, coordinating with team members, communicating with IDG, and supervising CNA's

Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate

Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtains/receives physicians’ orders as required

Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver

Document nursing care and significant interactions with patients at the time of visit; support team members

Administer medications and treatments as prescribed by the physician

Participate in follow-up with bereaved families with various types of contact

Discharge patients and coordinate transfer, including instructing patients/caregivers in follow-up care

Manage multiple patients and their support persons simultaneously in crisis with acute symptoms including pharmacolog

Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis

Responsible for ensuring and assisting in providing a safe and clean environment for patients and families

Assist in housekeeping needs that are compliant with infection control procedures and policies.
QUALIFICATIONS

Registered Nurse licensed by the State of Texas required

One year nursing experience required

Hospice/ Home Health experience preferred

Proficient skill using a computer is required; Microsoft Office Suite experience preferred

Must be comfortable/confident making independent decisions regarding patient care

Able to manage multiple tasks regarding immediate patient care
